Creating a return label requires (personal) information. Complete the form below and click the Send button. You will receive an email within one business day with instructions for shipping via UPS.

Once you have received the instructions by email, you can proceed with step 2 of the manual in the test kit.

The other materials needed to ship your medical sample are included in the test kit: a RigidSafetyBag and a return envelope coded UN3373.
